     Per saltire, Sable, Azure, Or and Argent;
Five chains bendwise in bend steel larger;
Five chains bendwise sinister in bend sinister steel
  larger;
In chief, a sail or in chief a longship or lower;
In dexter flank, a raven striking proper slightly
  to sinister;
In base, a bunch of grapes gules taller lower;
In sinister flank, a pine tree eradicated pine-green
  moved to dexter.

Notes

This is a better match than others I've seen. Most notably: the chains are aligned, tree is DARK green, longship has a sail (per GRRM), colors in correct order (Westeros is backwards).
